چکیده

A perforating vein can become incompetent due to hemodynamic or morphologic causes. Until recently all perforating veins with a diameter exceeding 3 mm at ultrasound exam were considered incontinent(1). Since introduction of duplex doppler and color doppler investigations this hypothesis was temperated because the dilatation of one or more perforating veins was usually observed in chronic venous insufficiency of the superficial veins, allowing the retrograde blood draining flow from the incontinent superficial veins to the deep venous system in standing position. These perforating veins substitute the normal function of the incompetent sapheno-femoral junction. In order to evaluate the incompetence of the perforating veins it is necessary to emphasize the valvular reflux from the deep venous system to a superficial vein(2, 3).

Surgical Anatomy of the Veins of the Lower Limb

With the advent of new surgical techniques, such as subfascial endoscopic perforator surgery, venous anatomy of the lower limb gained distinguished attention. Here, based on our surgical experience and observations on several dissections, we describe the embryology and the gross anatomy of the superficial, deep, and perforating veins. The lesser and greater saphenous veins, their named tributar...

Treatment of Venous Ulcer of the Lower Leg by Endovenous Laser Ablation

Venous ulcer is the most advanced stage of chronic venous insufficiency (CVI). Its prevalence is estimated to be between 1% and 3%. Recent articles have suggested that leg ulcers with reflux in superficial veins can be treated by endovenous laser ablation (EVLA). Endovenous laser ablation of the great and/or short saphenous vein has already become the treatment of choice for varicose vein and c...

Subfascial endoscopic perforator surgery for venous ulcers.

We report the treatment and outcomes of 12 patients who underwent subfascial endoscopic perforator surgery for severe chronic venous insufficiency and venous ulceration. All patients had received prior superficial venous ablative surgery and presented with incompetent perforating veins in the calf and persistent venous ulceration (lasting >10 years). Outcome measures included ulcer healing time...

Perforating Veins by Venography at Operation

All types of varicose veins can be treated successfully by a radical stripping operation in which the whole system of diseased veins and related perforating veins is removed (Myers, 1957; Arenander, 1960; Sherman, 1964). Simple procedures such as high ligation, with or without stripping of the main saphenous trunk, though widely practised, are much less successful, even in the early and uncompl...
